Centra program enhances patient care through technology [Video]

Centra has announced it is furthering its strategic theme of One Centra with the launch of its Remote Home Monitoring (RHM) Program.

This initiative, conceived during the challenges of the COVID-19 pandemic, aims to extend care directly to patients’ homes and redefine the patient experience, a news release states.

The RHM Program is designed to reduce hospital readmissions and emergency department visits while helping patients manage chronic conditions such as diabetes, congestive heart failure, pneumonia, chronic obstructive pulmonary disease (COPD) and hypertension.

Referred patients who opt to participate receive a comprehensive kit including a tablet, digital scale, blood pressure cuff, pulse oximeter and heart rate monitor. These devices connect via Bluetooth to a tablet, which automatically transmits data to Centra’s Command Center Team for continuous monitoring, the release states.

The tablets also remind patients to take their medications and complete daily surveys to assess their well-being.
