TOLEDO, Ohio (WTVG) – An oversight board has determined the cause of the explosion at the 2022 BP-Husky refinery that killed two workers
The U.S. Chemical Safety and Hazard Investigation Board released their final report Tuesday, finding that an overflowing drum of flammable chemical eventually led to the deaths of brothers Ben and Max Morrissey.
To prevent the chemical from flooding into other parts of the system, the liquid was released onto the ground, according to the report.
But then the vapors from the highly flammable chemical began to rise, eventually reaching an ignition point, which caused the explosion.
CSB determined there were insufficient safeguards to prevent overflow, and no procedures, written instructions or documented actions fixing that specific issue.
