• Develop a good home exercise program, utilizing resources such as MedBridge, constant therapy and other platforms to help transition the patient’s progress
• Engage the family in a comprehensive family training session
• Provide external resources in the community, i.e. support groups, service-connection, caregiver support, continued therapies
• Develop safe environment protocols for the patient and/or the family to implement at the house
